Club Forms- 

  • Active clubs must turn in both their Club Application/Information form & Club Constitution  annually  for ASB approval.  
  • Club Forms must be typed and printed.
  • Turn forms in to Mr. Isern in IC-10 or in his mail box in the office.

Procedures:

  1. Open up form by clicking on the link below.
  2. Fill out the form digitally.
  3. Print out the form, sign all required fields and turn in to ASB or Mrs. Luther accordingly for approval. 
Note:  Fundraising Request and Requisition originals must be printed and turned in in color. 
Note:  we suggest you SAVE AS... your Club Constitution Form, for future reference.

  • Fundraising Request Form (if you plan on raising money via an event/activity) 4 weeks Advance. NOTE we are currently experiencing problems with this form.  Please use the copy in the K-Drive under ASB>Current ASB Forms.  Sorry for the inconvenience as we solve the glitch.
  • Requisition Form (if your club wants to spend money) 2 weeks in advance.
  • Change Request Form (if your club needs starting money for en event) 1 week advance.
